ASSUMPTION OF RISK

I, the undersigned coach, acknowledge that participation in baseball/softball training

activities at The Yard Performance Center involves inherent risks. These risks include,

but are not limited to:

 Being struck by balls, bats, or training equipment

 Slips, trips, and falls

 Collisions with other participants or objects

 Physical exertion that may result in injury

I understand these risks may result in serious injury, permanent disability, or death.

RELEASE OF LIABILITY

On behalf of myself, my team, and all participating players, I hereby release, waive, and

discharge The Yard Performance Center, its owners, employees, trainers, and

affiliates from any and all liability, claims, demands, or causes of action for injuries,

damages, or losses arising out of or related to participation in activities at the facility.

COACH RESPONSIBILITY & AUTHORITY

I certify that:

 I am the authorized coach/representative of the above-listed team

 I have permission from all players and their parents/guardians to participate

 I have communicated the risks associated with participation to all players and

parents/guardians

 I accept full responsibility for ensuring my team follows all facility rules and safety

guidelines

MEDICAL AUTHORIZATION

In the event of an injury or medical emergency, I authorize The Yard Performance

Center staff to obtain emergency medical treatment for any player if necessary. I

understand that I am responsible for any medical costs incurred.

INDEMNIFICATION

I agree to indemnify and hold harmless The Yard Performance Center from any claims

brought by players, parents, or guardians associated with my team.

RULES & CONDUCT

I agree that my team will:

 Follow all facility rules and instructions

 Use equipment properly

 Respect staff, other participants, and the facility

Failure to do so may result in removal from the facility without refund.

PHOTO/VIDEO RELEASE (Optional)

I grant permission for The Yard Performance Center to use photos/videos of team

activities for promotional purposes.

24-HOUR CANCELLATION POLICY

I understand that cancellations must be made at least 24 hours in advance. Failure to

do so may result in forfeiture of scheduled time or fees.

ACKNOWLEDGMENT

By signing below, I acknowledge that I have read and fully understand this waiver and

agree to its terms on behalf of my entire team.
